Why Should You Partner With Empire State Health Partners ACO?

Payment has changed. Working with Empire State Health Partners positions you for success within a payment model determined by quality scores and relative costs of your patients.

To achieve this benefit, we will work with your office staff to accomplish the following:

Doctor speaks with patient while looking at clipboard

Quality scores in the 80th to 90th percentile.

Promote annual wellness visits.

Gather data to understand where your patients are more expensive than regional and national averages (e.g., certain diseases, post-acute care, drugs, DME, etc.) and deploy strategies to constrain those costs.

Coordinate care for your chronically ill patients.

Qualify for Patient Centered Medical Home Certification.

Get complete claims data from all sources on your Medicare patients so that you better understand the full spectrum of health care services they use and frequently do not report to their physician.

Get drug refill information on your patients.

Gain access to similar shared savings programs with other payers.

Be as ready as possible for future value-based payments.
