About Empire State Health Partners ACO

Empire State Health Partners ACO can make a difference for small independent practices working with an ACO. We have developed and worked with a physician-led ACO that helps medical providers to stay in Medicare compliance, stay in control, and stay in business! Our goal is to help our members maintain their independence and financial stability. We help practices stay informed about recent changes such as the Centers for Medicare & Medicaid Services’ 2030 objective to have 100% of Medicare patients in an ACO.

A benefit of belonging to an ACO is that it provides financial incentives for delivering quality care and complying with cost reporting requirements. We strive to keep practices informed about changes in payment models, making the transition to new procedures easier.
