About ACOs

At ESHP ACO, we make it our mission to make Medicare regulatory compliance simpler for physicians. We want to maintain the vision of independently practicing and financially stable physicians in a rapidly shifting regulatory environment, long into the future.

We recognize that currently, it is becoming less and less possible for physicians to maintain independence and profitability, as venture capitalists seek to take over practices overwhelmed by regulatory red tape.

More Information About ACOs

ESHP ACO is different. We want to help you practice medicine the way you always have, with the freedom to spend the appropriate amount of time with each patient to address their medical needs. We want be the partner you turn to when you have questions about requirements, and we want to do the hard compliance work for you, so you can focus on treating your patients.

What is an ACO?

ACOs are groups of doctors, hospitals, and other health care providers who come together to give coordinated high-quality care to their Medicare patients. The goal of coordinated care is to ensure that patients get the right care at the right time while avoiding unnecessary duplication of services and preventing medical errors.

When an ACO succeeds in both delivering high-quality care and spending health care dollars more wisely, it will share in the savings it achieves for the Medicare program.

The Advantages of Joining an ACO

We understand that the existing standards, while accommodating larger corporations, are not fair to smaller practices, the physicians in these practices, or their patients. Empire State Health Partners ACO helps make things fairer and helps physicians afford to keep their practices. Making the strategic decision to become part of an ACO provides practices with a wide range of immediate benefits as well as future leverage. These advantages include:

  • Increased practice revenue
  • Credit for MIPS reporting
  • Forging relationships with other independent providers in the area
  • Access to HealthEndeavors data analytics

Bringing Value-Based Care to Independent Physician Practices

Today, we are working together to reshape the way care is delivered by placing more value on patient satisfaction, improved outcomes, and healthy living — all while lowering the cost of care. Our network consists of some of the region’s best providers who have chosen to join together to transform our community’s health care.
